Pre-Trip Forms

In order to ensure that each of our clients have a rewarding trip, they must be safe while on the trip, and come home in one piece. To do that, some basic information is needed.

There are three forms every participant must fill out prior to traveling with Volant.

1. Personal Contact Information form
2. Release of Liability Form
3. Medical History Form

Each form has a specific purpose and should be completely filled out by the participant.

Be as forthcoming as you can, to not only ensure your safety, but to ensure the safety of those you will be travelling with.
